Site Visit Reflection

My site rotation evaluations for LTC were with Dr. Davidson. Since I was not technically at a long term care facility, and was mainly doing internal medicine, I did not really know how to write a LTC-style H&P. Dr. Davidson was extremely helpful in teaching us to write in that style. Even before our rotation, in our week-long online rotation, she gave us practice, and before our first site visit, she sent us the format to help us write the H&P’s correctly. Then, she was very helpful and thorough with her feedback, both during the site visit and afterwards with sending the corrected H&P. Based on her feedback and comments, I was better able to write my future H&P’s with the correct techniques in a way that really explored and understood the patient problems and how to most effectively treat them. 

For my first H&P, I presented a patient with dementia who came in with suspected stroke symptoms. It was difficult to get a detailed history, and Dr. Davidson helped me understand where I should have been thorough in understanding his issues and specific tests I should do based on his symptoms. For my 2nd site visit, I presented an H&P of a patient who came in with SOB and cough. I was able to do a much better job thanks to Dr. Davidson’s useful feedback. 

For our Pharm Cards, Dr. Davidson encouraged us to point out specific “pearls” and important information to remember regarding the specific drugs. I found this very useful since those usually include specific contraindications, adverse effects, things to monitor, and other important points which must be kept at the top of our mind when giving specific medications, so it was helpful to highlight them.

Overall, I really enjoyed these site visits with Dr. Davidson. I really learned a lot, and even learned more through further discussions about topics that had come up during the visit.
